The Diabetes Total Lifestyle Change (DTLC) Program is a comprehensive 12-week virtual initiative designed to support individuals in managing and preventing type 2 diabetes, prediabetes, and other lifestyle-related conditions. Our mission is to empower participants with the knowledge, tools, and support needed to make lasting health improvements.
Engage with a multidisciplinary team of health professionals, including endocrinologists, clinicians specializing in diabetes care, psychologists, registered nutritionists and dietitians, and podiatrists.
Our curriculum encompasses nutrition education, physical activity guidance, glucose monitoring techniques, and behavioral coaching to address the multifaceted aspects of diabetes management.

90-minute sessions held once a week via Zoom, led by a team of experts in nutrition, diabetes care, physical activity, and mental health.

Participants join a private WhatsApp group for peer motivation, weekly check-ins, challenges, and access to expert tips and reminders throughout the week.
You’ll receive recorded videos, handouts, grocery lists, fitness suggestions, and lifestyle tracking tools to apply what you learn in real life.
Participants are encouraged to track weight, blood sugar, food choices, and physical activity to observe real progress.
After 12 weeks, participants graduate and are offered opportunities to mentor others, join alumni networks, or continue advanced wellness journeys.
